Perhaps he was baptised as John Thomas Caine and later on, for whatever reason, wished to use the name Stanley as a middle name rather than Thomas ---- eg perhaps a Stanley or a Mr Stanley had had a very strong influence on his life during childhood/adolescence. Perhaps he always used just John or John Stanley, and it was his family who used the initial T in his MI.

Or maybe he didn’t like having exactly the same name as the sibling who had died as a young child so chose a new middle name instead.

Or maybe he always was John Thomas Stanley Caine and used whichever of his middle names he preferred at any given time.

It would be interesting (but expensive) to look at all 3 birth certificates of the children whose father’s name was given as John Stanley Caine, to see whether he’d registered those births himself, or whether Nellie had registered them.
