Well I now have some info from Ballamona hospital. A lot is of course medical terminology which I can't understand plus the fact several words are abbreviated to 2 or 3 letters. The photocopies are under 50 pence a sheet which is a good price. Some details I could not receive as they were considered confidential. To sum up, I found out a lot I didn't know but still I have many questions that were not answered. Extremely poor handwriting on the forms & a lot of medical words. The woman on the IOM who sent me the forms was very helpful .
