If someone was on the Maughold burial register they were buried in the parish church grounds of Maughold.

Maughold and Lezayre are parishes, and Ramsey a town which straddled part of both, as Nigel described. But there is a large part of Maughold and Lezayre which is not Ramsey! So Maughold was not synonomous with Ramsey any more than Queensland is synonomous with Brisbane.

Read Manx Notebook for a description of the Ramsey churches, and the reason why many people were buried in Maughold (or Lezayre) rather than Ramsey.
Ramsey burials are not included on Brian Lawson's burial index because they have not been transcribed by the FHS, but you can see them on LDS film.
